This notice is provided under the Privacy Act,
5 U.S.C. 552a(e)(3), and concerns the information requested on the user account to
access the Security and Hazardous Materials Safety (ASH) Customer Satisfaction Survey (CSS Survey).
Authority: Information
solicited by the Federal Aviation Administration (FAA) ASH CSS Survey is collected under the authority of 49 U.S.C. 329(b) and 6 U.S.C. 1523.
Purpose: The principal
purpose of collecting your Personality Identifiable Information (PII) is to
create a new user account to access the FAA ASH CSS Survey.
Routine Uses: The information
collected will be included in the system of records notice DOT/ALL 13, Internet/Intranet
Activity and Access Records, 67
FR 30757 (May 7, 2002) and is subject to the published routine
uses, including:

Act system of records. These routine uses are published in the Federal Register

Disclosure: Submission
of the information is voluntary; however, failure to submit the requested
information will result in FAA s inability to grant you access to the system to
take the survey.
